Usually isn’t a correlation between the degree of slip and severity of the symptoms.
Nerve root irritation can occur if there is encroachment occurring due to neural arch rotation or anterior osteophytes.
These symptoms are exacerbated by lumbar extension and rotation.
Chronic mechanical midline pain that is usually alleviated by pain.
Radicular type pain can occur down the leg and is more common in higher graded slips.
Strength and sensation are usually normal.
If hyperreflexia is noted, the spine should be assessed to rule out spinal cord lesions or cauda equina.
Bicycle test may help to distinguish from vascular or neurogenic claudication.
